Dear Right to Information & Privacy Officer,

I am writing to make a formal application for access to information under:

  • Section 40 of the Information Privacy Act 2009 (Qld) – to access my own personal information; and, if necessary,

  • QPS Client Reference Number (if known): [Insert or “unknown”]

  • Proof of identity: A certified copy of my Queensland driver licence is attached.

2. Information Requested

I request access to all Queensland Police Service records concerning an incident in which I was alleged to have contacted Triple Zero (000) and “behaved strangely.” Specifically, I seek:

a. Computer Aided Dispatch (CAD) Records:

  • Originating telephone number (CLI) as recorded by Telstra / ESTA

  • Transcript or audio recording of the call (or confirmation if unavailable)

b. Operational and Incident Reports:

  • Any Event Report or QPRIME Occurrence, including any narrative generated in response to the call

c. Officer-Generated Records:

  • Body-worn camera (BWC) video

  • In-car video

  • Notebook entries from attending officers

  • Any other documentation or media

d. Internal Communications:

Relevant timeframe: 1 January 2024 – 30 June 2024

Relevant locations: Kalbar Police Station, Harrisville Police Station, Ipswich District Communications Centre

3. Purpose of Request

The information is sought to verify the accuracy of the allegation and, if necessary, to correct any inaccurate records and pursue appropriate complaints or follow-up action.

4. Form of Access Requested

Preferred format:

  • Documents: PDF

  • Video: MP4

  • Audio: WAV or MP3

Delivery via secure email link or USB mailed to the address above is preferred.

5. Fees

As the request primarily concerns my personal information, no fee is required under the Information Privacy Act.

If any parts of the request fall outside the scope of the IP Act, please process them under the RTI Act. I am willing to pay the $55.75 RTI application fee upon invoice. Please notify me in advance of any additional processing or access charges.

6. Contact Details

Should clarification be required, please contact me at [email address], or by post if necessary.

Thank you for your assistance. I look forward to your written acknowledgment and processing of this application in accordance with statutory timeframes.

Yours faithfully,

(signature if posting)

Frank Russell
