Dear Services Australia FOI Team, Manager Sean, NDIA and Oversight Representatives,

RE: LEX 93986 — REQUEST FOR ORIGINAL AUDIO, TRANSCRIPTS AND DECISION RECORDS

I acknowledge the release of documents dated 3 November 2026 under LEX 93986.

The release confirms the existence of a Management of Services Profile affecting my telephone and face-to-face access to Services Australia.

The released material also records an earlier referral from the FOI team concerning excessive correspondence.

However, significant questions remain unanswered.

1. WHERE ARE MY VOICE RECORDINGS?

I formally request access to the original, unedited audio recordings of my telephone interactions with Services Australia, particularly:

- My conversation with Manager Sean on 19 October 2026.

- Calls concerning the creation, continuation or review of my Management of Services Profile.

- Calls relating to Rent Assistance cancellation, review requests and complaints.

- Calls during which restrictions or alleged behavioural incidents were discussed.

2. WHERE ARE THE TRANSCRIPTS?

Please provide all existing transcripts, speech-to-text records, call summaries and contemporaneous notes associated with those recordings.

If no transcript exists, please confirm this rather than substituting a caseworker's summary for a verbatim record.

3. WHAT EVIDENCE SUPPORTED THE RESTRICTIONS?

Please identify and provide the original incident reports, correspondence, decision records, authorisations and reviews relied upon to classify my profile as Reactive and restrict my access to ordinary service channels.

Where allegations concerning my behaviour were recorded, I request the underlying primary evidence so those allegations can be assessed fairly.

4. PRESERVATION OF RECORDS

Please preserve all relevant original audio files, call metadata, case notes, transcripts, system audit histories and associated communications pending completion of my FOI requests and any review proceedings.

5. CROSS-AGENCY RECORDS

NDIA FOI 26/27-0675, Part 3, page 197, records an explanation linking my Services Australia restriction to FOI activity.

Please identify the original source of that explanation and any documented communication between Services Australia and the NDIA concerning my service restrictions.

6. FORMAL FOI ACCESS REQUEST

To the extent these materials are not already covered by LEX 93986, LEX 95192 or other pending applications, please treat this correspondence as a new request for access under the Freedom of Information Act 1982.

Please provide a new reference number, identify any overlapping requests and advise whether any recordings have been deleted or are scheduled for deletion.

I am not withdrawing or narrowing any existing application.

The agency has now disclosed its administrative conclusions. I am requesting the original evidence behind those conclusions.

I ask for written acknowledgement and a substantive response.

Kind regards,

Thomas Brecelic
