Truth Sequences: Thirteen Years of Administrative Decisions

A cross-agency evidence investigation, 2013–2026

Central question: What happens when a person receiving disability support repeatedly challenges decisions affecting their income, employment, medical care, banking, communication and personal liberty?

This dossier examines a long history of complaints and administrative decisions. It does not assume that every organisation acted together. It asks whether errors, restrictions, inaccurate records or information-sharing created a cumulative pattern of disadvantage.

The evidence pathways

Employment services: APM compliance decisions, demerit reversals, employment assistance, vocational support and complaints to DEWR.

Income support: Disability Support Pension interruptions, Rent Assistance decisions, payment reporting, reviews and compensation requests.

Financial access: Bank account restrictions, card access, deductions and the consequences of being unable to access funds.

Telecommunications: Dodo service termination, notification records and access to essential communication.

Healthcare: Psychiatric treatment decisions, medication continuity, clinical reports and the relationship between interrupted services and disability support needs.

Police and enforcement: Reported Taser deployment, police interactions, use-of-force records and the relationship, if any, to administrative restrictions.

Services Australia: Managed Service Plan decisions, FOI-team referral, police-warning correspondence, restrictions on office attendance and original notification records.

NDIA and NDIS providers: PACE records, support coordination, vocational recommendations, housing support, service transitions and complaint handling.

Documentary anchors

- APM compliance events dated September 2023 and January 2024, subsequently reversed.

- Services Australia FOI LEX 93986, including April 2025 restriction records and a documented FOI-team referral.

- NDIA FOI 26/27-0675, including the August 2026 record discussing Services Australia restrictions.

- Michael Lewis's June 2026 occupational therapy report, documenting functional support needs and vocational goals.

What the investigation must establish

For every adverse decision, identify the decision-maker, original evidence, statutory or contractual authority, notice, opportunity for review, system records and resulting consequences.

For every suspected cross-agency connection, establish whether information was actually transferred, who accessed it, what purpose was recorded, and whether it was accurate and lawfully handled.

For every alleged loss, identify the dates, amounts, supporting records and available remedies.

The accountability question

Were the rights, dignity and reasonable-adjustment needs of a disability pension recipient adequately protected throughout these processes?

This is a request for transparent, independently verifiable answers—not a presumption of conspiracy or misconduct.
