Dear APM Privacy Officer,
I am requesting access to personal information held by APM Employment Services concerning two compliance demerits recorded in September 2023 and January 2024, which I understand were subsequently removed.
Please provide copies of:
1. The original demerit records, including event dates, appointment or obligation details, reason codes and the staff or system responsible for recording them.
2. The decisions and reasons for subsequently removing, reversing or otherwise changing each demerit.
3. Audit histories showing the dates and times of those changes, including available user or system identifiers.
4. Relevant case notes, provider correspondence, appointment records, notifications, internal reviews and communications with the responsible government department.
5. Any records identifying whether either demerit affected my income-support payments or compliance status.
I also request clarification of the statement that both demerits were removed while no further information could be provided.
If APM no longer holds the relevant records, please identify the applicable retention arrangements and which government department or records custodian may hold them.
Please treat this as a formal request for access to my personal information under applicable privacy law, and as a complaint about the absence of a substantive explanation.
I request a written acknowledgement, reference number and response through email.
